Pre Appointment Letter
Due to the COVID-19 global pandemic, we have modified our check in process in order to help protect our staff and patients. We request that each patient complete this form prior to his/her dental visit. We ask that you complete this form prior to arriving at our office. We will not be able to accommodate your appointment without having received this before you arrive for your appointment. A new form must be completed for EACH dental visit. COVID-19 Screening Form
When you arrive for tomorrow’s scheduled appointment, we will follow these procedures:
Please arrive at least 10 minutes PRIOR to your scheduled appointment. If you arrive more than 15 minutes AFTER your scheduled appointment, your appointment may be rescheduled.
Once you have parked in our parking lot, please call us at (408) 374-2745 to let us know you have arrived.
Please remain in the parking lot and we will call/text you when we are ready for you to enter our office.
Please wear a face mask or covering. We will ask you to use hand sanitizer upon entering our office.
You will be asked the same questions that were asked in our “Pre appointment screen form” or you may be asked if there have been any changes since you completed the form.
You will have your temperature taken with a touchless thermometer and your oxygen level will be taken with a pulse oximeter.
If you experience any COVID-19 symptoms within the next 14 days of your appointment or test positive for COVID-19, please inform our office.
Please be aware that if you develop any of the following symptoms/warning signs for COVID-19, you should seek medical attention immediately:
Persistent pain or pressure in the chest
New confusion or inability to arouse
Bluish lips or face
